We have an exciting opportunity for a motivated and enthusiastic Support Worker to join our Learning Disability Team based at Bickerstaffe House.
This rewarding role will involve providing short term, person-centred support to adults with a learning disability living in the community.
You will help people achieve their identified outcomes, develop independence, reduce risks and access opportunities that improve their wellbeing and quality of life.
As part of a busy and supportive team, you will also play a key role in supporting our Front Door Duty function and your work will help ensure vulnerable adults receive timely support and that risks are identified and addressed at the earliest opportunity.
- Delivering short-term interventions to adults with a learning disability.
- Supporting individuals to develop daily living and independent living skills.
- Promoting choice, control, inclusion and community participation.
- Undertaking welfare and wellbeing calls to people.
- Supporting the team's Front Door Duty service.
- Liaising with families, carers, providers and partner agencies.
- Maintaining accurate records and contributing to care planning and reviews.
- Supporting individuals to access local community resources and opportunities.
