About the Role

In this rewarding role, you will support vulnerable people to maintain and regain their independence, enabling them to live safely and comfortably in their own homes. Using a strengths-based approach, you will help individuals improve their quality of life and achieve their personal goals.

You will also play a key part in supporting people as they transition home from hospital, working within Discharge to Assess pathways, as well as providing complex and palliative care support where needed.

Previous experience in care is beneficial but not essential—we welcome applicants with the right values and a willingness to learn.

Working Hours

This role involves working on a rota basis, including weekends, bank holidays, and shift patterns.

About You

If you are someone who is compassionate, motivated, and eager to make a meaningful difference, we would love to hear from you. A full driving licence and access to a vehicle is desirable.

As a front line care provider we ensure Government guidance is implemented and practiced at all times with regards to the current climate.

At this point in time we are not able to offer a Visa sponsorship in this role due to operational reasons.

We are committed to safeguarding and promoting the welfare of children/vulnerable adults. This post is subject to satisfactory reference history of at least two years, Disclosure & Barring Service (DBS) check, medical clearance, evidence of any essential qualifications and proof of Right to Work in the UK in accordance with the Asylum and Immigration Act 1996.

This post is exempt from the Rehabilitation of Offenders Act and amendments to the exemptions order, which mean that certain offences are ‘protected’ and do not need to be disclosed, guidance on the "filtering" of offences can be found at.

The successful applicant will be required to work outside of a 7am to 8pm Monday to Friday work pattern and will receive an additional payment on top of the quoted salary as a result.
