Overview
An exciting opportunity has arisen for a Case Assessor to join our Learning Disability Team, based at Bickerstaffe House. This role is ideal for someone with experience in adult social care who is committed to delivering high quality, person centred support for people with learning disabilities.
Responsibilities
- Complete Care Act reviews for adults with a learning disability.
- Work with individuals, carers, providers and partner agencies to ensure care and support plans remain appropriate, outcomes focused and proportionate to need utilising a strengths based approach.
- Manage a caseload, ensuring reviews are completed in a timely manner and in line with the Care Act 2014.
- Identify changes in need, recommend adjustments to support packages where appropriate and contribute to effective care planning.
Qualifications & Experience
- Strong communication skills.
- Good understanding of the Care Act.
- Experience of working with adults with learning disabilities.
