About the Role

You will be part of a multi-disciplinary team working collaboratively with health and care professionals across Blackpool. Your work will support adults and carers to remain safe, well, and as independent as possible within their own homes and communities. This is a varied and rewarding role, involving:

  • Care Act 2014 and Mental Capacity Act 2005-compliant assessments
  • Safeguarding practice
  • Active participation in multi-disciplinary meetings
  • Coordinated care planning and timely discharges
  • Working alongside hospital and community health colleagues
