Blackburn with Darwen Council is recruiting a passionate Social Worker to join the Hospital Discharge Team. You will work with multi-disciplinary teams to assess social care needs, prevent unnecessary hospital admission and support safe and timely discharges in a Discharge to Assess setting.
The role requires undertaking statutory social work functions in line with the Care Act and the Mental Capacity Act, on a 7-day rota including weekends, and delivering person-centred care that promotes independence.
