The Hospital Discharge Team consists of Qualified Social Workers who work closely with the multi-disciplinary teams, using a Home First Approach across the wards and wider community. We are looking to recruit a passionate and focused Social Worker to come and join our fantastic team, providing a social care service for the residents of Blackburn with Darwen, to prevent hospital admission, facilitate safe and appropriate discharge, and undertake the social care assessment within a Discharge to Assess setting. This will include working on a 7-day rota basis covering weekends.
The Hospital Discharge Team supports to facilitate safe and timely discharges from hospital. To achieve this, we use a wide variety of personalised support, assist people to promote independence, support people with complex needs to ensure the maximum level of choice and control available to them, and promote the principles of The Care Act and the Mental Capacity Act. We focus on how people can be supported by their strengths, their networks and the local community. Where care is required, we work to ensure that this reflects the individual's assessed needs and strengths, while promoting their independence, choice and control.
You will be expected to undertake statutory Social Work functions in line with the Care Act and the Mental Capacity Act. To undertake duties expected of Social Workers in line with the Professional Capabilities Framework and in accordance to the job description attached.
