Job overview
This is an exciting opportunity to join a 16-bed female acute ward. The post holder carries responsibility and accountability for the health care management of their patient group, in collaboration with senior nurses and Team Leaders. The band 5 staff nurse will be responsible for assessing, developing, implementing and evaluating family centred programmes of care.
Main duties of the job
Maintain high standards of nursing care and promote an environment suitable for our patients, ensuring that all planned care fully reflects latest research developments. To assess, develop, implement and evaluate person centred care in collaboration with the patient, their family/carers and with colleagues or senior staff and ultimately without direct supervision. Actively participate in mentorship for junior staff and students and participate in clinical supervision. Ensure safeguarding issues are acted upon promptly and appropriately. Continuously develop the clinical role in order to be able to work independently, anticipate care needs of patients and actively manage their journey through recovery. This includes being able to work independently or as part of a team in potentially stressful, distressing or challenging circumstances. To extend and expand nursing roles within the nurses competence, and with appropriate training as guided by local and departmental policies and procedures.

1. Communication/Relationship Skills
•Maintain confidentiality when dealing with patient or sensitive staff information. Follow relevant guidelines and exercise sound judgement when accessing/disclosing information.
•Communicate in an appropriate and professional manner.
•Maintain clear, accurate clinical records in accordance with professional standards.
•Develop collaborative relationships with patients and carers to promote engagement in the therapeutic process, adapting to barriers to communication as required.
•Co-ordinate and communicate complex care packages via CPA, reports and documentation, liaising with other agencies where appropriate.
•Work collaboratively with the wider MDT.
•Work within professional boundaries, with reference to professional guidelines and local policy.
2. Knowledge, Training and Experience
•RMN (Degree or DipHE)
•Up to date knowledge of Mental Health Act, NMC Code of Conduct, and other relevant legislative and policy requirements and guidelines.
•Engagement in clinical and management supervision.
•Responsible for maintaining registration and meeting revalidation requirements.
•Demonstrate a clear understanding of CPA and promote best practice within your own team.
•Skilled in assessing, planning, implementing and evaluating care and treatment.
•Demonstrate consistently the ability to engage with people experiencing acute mental health problems.
•Clear understanding of risk and clinical risk assessment.
•Work to a high standard under pressure, prioritising tasks and managing time effectively.
•Develop/demonstrate self-awareness, seeking space, support or relief if needed.
•Develop the skills to co-ordinate and lead a team on a shift by shift basis.
•Develop de-escalation skills working with distressed people.
•Recognise safeguarding concerns and respond appropriately.
3. Analysis, Judgement and Problem Solving
•Use professional knowledge and clinical judgment to assess clinical situations and utilize problem solving skills, observing legislative and professional requirements and without acting outside of Trust policy.
•Work with other disciplines, acute services and partner agencies to achieve the best outcomes for patients.
•Attend and contribute to team meetings, reflective practice, MDT reviews.
•Demonstrate skill in effective decision making appropriate to position, potentially under challenging circumstances.
•Work towards agreed clinical objectives, utilizing support and resources as required.
•Make decisions regarding prioritization and utilization of ward resources appropriate to position.
•Review and evaluate efficacy of clinical interventions.
4. Planning and Organisational Skills
•Prepare adequately to communicate patient information concisely and accurately to incoming staff, or to other professionals as required.
•Co-ordinate shifts, planning and delegating effectively to ensure safety is maintained, essential tasks are undertaken and effective care is delivered.
•Plan and detail care collaboratively, using SMART goals where appropriate, to meet patients’ needs.
•Report environmental issues, low stock or other operational concerns appropriately to maintain functioning.
•Refer to regular audits to ensure standards are being met, seeking clarification around any points which aren’t clear and addressing identified omissions promptly.
•Support senior staff by completing simple audits as requested.
•Ensure practice is evidence-based and maintain an up to date knowledge of best practice and research developments
•Order, store, record and administer medication appropriately in accordance with Trust policy and professional guidelines.
•Provide patient-centred, recovery focussed, holistic care using the APIE process.
6. Policy and Service Development
•Contribute to the development and maintenance of the standard in the ward area.
•Operate within Trust policies and procedures and adhere to discipline specific clinical guidelines/ professional codes of conduct.
•Use national guidelines, Trust policies and professional codes to safeguard standards of care for patients, carers and colleagues.
•Be aware of current issues in Urgent Care and development of Trust policies and share these with colleagues.
•Support service improvement initiatives and the management of change.
•Report clinical incidents appropriately.
