The Housing Enablement Team is looking for a Hospital Housing Triage Officer to support the timely triage, coordination and administration of housing-related referrals linked to hospital discharge and community support. This role will act as the first point of contact for the Housing Enablement Team, managing the duty phone and inbox, receiving referrals and enquiries from partners, medical professionals and other agencies.
You will gather relevant information, record accurate case notes, screen and triage referrals in line with agreed processes, and ensure matters are directed to the appropriate officer where further support is required. You will also provide essential administrative and coordination support to the team and Service Manager, helping to maintain accurate records, update spreadsheets and service data, support performance monitoring, arrange practical discharge-related support, and assist officers in hospitals or community settings when required.
You will be expected to communicate professionally and sensitively with partners, patients, family members and professionals, particularly where individuals may be vulnerable, distressed or experiencing complex housing circumstances. You will also need to be confident handling confidential information, escalating concerns appropriately, and supporting the team to deliver safe and timely outcomes.
Qualifications
- A good working knowledge of Word, Excel and SharePoint, with administrative experience.
- The ability to work with minimum supervision and prioritise own workload.
- Experience of dealing with enquiries by telephone and email.
- The ability to communicate sensitively and professionally with patients, families, professionals and partner agencies.
- Experience of working in a pressured environment.
- Previous experience of working with vulnerable people, or a clear interest in developing within this area.
Additional Requirements
This post will require an Enhanced Disclosure and Barring Service check (DBS check). All offers of employment will be conditional upon receipt of a satisfactory DBS check. You will also need access to a vehicle, a full driving licence and business insurance to travel across Leicester, Leicestershire and Rutland as required.
Scope of Service
The role supports the Housing Enablement Team across the three UHL hospitals – Leicester Royal Infirmary, Glenfield Hospital and Leicester General Hospital – as well as community hospitals across Leicester, Leicestershire and Rutland, the Bradgate Mental Health Unit, mental health rehabilitation units and Mental Health Services for Older Persons wards. You may therefore be required to visit these sites to support service delivery, attend meetings or assist with events.
