Birmingham Mind is seeking an Admissions Avoidance Coordinator to work with young adults aged 18–25 who are supported by BSMHFT services. You will deliver person-centred support, focusing on preventing hospital admissions and strengthening resilience.

You will collaborate with Discharge Coordinators and CYP staff, using needs and risk assessments to tailor interventions in the context of social determinants of mental health. A full UK driving licence is desirable but not essential.
