Band 5 Community Staff Nurse Evening Service
There are 3 posts available offering up to 22.5 hours a week as a maximum. The Evening Service operates from 6:00 pm to 10:30 pm, seven days a week, with weekend and bank holiday work and flexibility on the number of evenings to be worked. The centralised hub is based at Priestley Wharf and the service covers the city of Birmingham. This role requires a high degree of mobility and a full driving licence with access to a car for work. Training and development will be provided to support successful candidates in achieving competence in relevant areas.
The role involves working with a multidisciplinary team to support complex patients in the home setting, with a focus on nursing duties as part of holistic care management and assessment of health needs in areas such as palliative care, wound management, diabetes and other long-term conditions. The evening service visits both planned patients and emergent visits (e.g., palliative patients requiring support).

Responsibilities
- Carry out a variety of nursing duties as part of holistic care management through assessment and evaluation of health needs.
- Support patients to maintain independence and well-being in the home setting.
- Work as part of the multidisciplinary team to achieve the best outcomes for patients.
- Provide care during evening service visits, including planned and emergent visits.
- Engage in training and development to achieve competence in relevant areas.
Qualifications/Requirements
- Registered nurse (Band 5) with current registration (NMC or equivalent).
- Full driving licence and access to a car for work.
- Flexible to work evenings, weekends and bank holidays as required by the service.
- Experience in palliative care, wound management, diabetes or other long-term conditions is beneficial.
- Strong communication and teamwork skills.
