In this role you will be the first point of contact for individuals and families, delivering holistic, person‑centred support that builds resilience and reduces reliance on statutory Adult Social Care. You will manage caseloads and work collaboratively with local partners to coordinate tailored interventions that meet diverse needs.

  • Identify and respond to safety and safeguarding concerns in partnership with services
  • Map and understand local community strengths, resources and gaps to promote inclusivity
  • Maintain accurate records, collate data and use Microsoft tools effectively
  • Build and sustain strong working relationships with colleagues, partners and community assets
  • Advise on local issues affecting citizens with diverse needs to inform service planning

What We’re Looking For

  • Inclusive and empathetic, with a commitment to diversity, equality and inclusion
  • Creative problem‑solvers, able to act decisively and compassionately
  • Effective communicator, strong interpersonal and communication skills
  • Collaborative team player, experienced in working with local services and communities
  • Flexible and adaptable, willing to work across locations and support evening or weekend delivery
  • Safeguarding‑aware, with a strong understanding of risk, vulnerability and support pathways
