An exciting opportunity has arisen for a full time Band 7 Advanced Nurse Practitioner to join our North Birmingham Neighbourhood Mental Health Team, working with our GP partners. Our community mental health services are well-established teams and as part of the BSOL community transformation programme, we are looking to increase our primary care teams with these roles. This is an exciting opportunity to be at the heart of national change and developments in the delivery of community mental health care. The development of our Primary Care mental health team is an essential service which we are in the process of evolving in line with the NHS Long Term Plan.
Main duties of the job:
- Work within a multi-disciplinary team, striving for the best outcomes for individuals within the local community.
- Provide mental health triage, assessment and outcomes based intervention for service users including nurse prescribing within the Primary Care Mental Health Team.
- Identify need and best course of advice, support or intervention.
- Promote psychosocial wellbeing by identifying and supporting pathways for service users and carers.
- Ensure robust communication across BSMHFT and primary care, providing support and advice to all colleagues with the PCN as required.
- Participate within the local neighbourhood MDTs.

Person Specification:
- Experience as Advanced Nurse Practitioner.
- Hold a Nurse Independent Prescribing qualification.
- Experience in Community Mental Health Service.
- Experience of working within primary care.
- Experience of working with other VCFSE.
