Band 6 ASD/LD Practitioner โ SOLAR Crisis Team
We are seeking a Specialist Practitioner for the assessment and co-ordination of the care needs for Young People under the care of SOLAR/MPower with a ASD/LD diagnosis.
- To plan and implement evidence-based care/interventions for Young People with a diagnosis of ASD/LD who are at risk of admission to hospital.
- To contribute to the CETR process giving expert advice/guidance to plan the best treatment(s) for/with the Young Person.
- To attend ward reviews for those young people under M-power with a diagnosis of ASD/LD to help plan and facilitate their discharge from an inpatient setting.
- To promote safeguarding and the welfare of the young people neurodevelopment conditions and families.
- To work as part of the multi-disciplinary team providing professional advice and support to both colleagues within and outside of the service.
- To work in the team approach and support colleagues when required.
- To act as a role model for students and junior staff.
- To support colleagues within the wider service.
- Will have highly developed skills in assessing the needs of young people with neurodevelopmental conditions and their carers based on the analysis of highly complex facts and situations.
- Will contribute to the CETR process giving expert advice/guidance to plan the best treatment(s) for the Young Person.
- Will work with Young People and their families, liaising with external agencies.
- Will actively promote user/carer involvement in all aspects of care planning/delivery whilst respecting the rights, capabilities and wishes of the individuals concerned.
- Will undertake highly specialist assessments using and interpreting standardized assessment tools and clinical judgment in line with evidence based practice.
- Will develop and maintain therapeutic relationships with service users and carers.
