We are looking for a colleague to join our Practice Team, making a difference to patients in Letchworth. You will be qualified and have worked as a practice nurse in primary care, engaged in regular nurses clinics. You would be joining us and work as a member of the practice multidisciplinary team, ensuring nursing services are delivered effectively. The Practice Nurse will work in a number of clinical areas such as infection prevention and control, health promotion, chronic disease management, well women and well man clinics, as well as actively supporting the practice management team in the reviewing and delivery of clinical policy and procedure.
Main duties of the job:
- To support the Infection Prevention Control (IPC) lead and to work to the IPC Policy supporting the implementation of audits, training, support and evidence of compliance to the IPC lead and practice manager.
- To help develop, implement and embed an effective care management programme for frail and housebound patients.
- To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice.
- To develop, implement and embed health promotion and wellbeing programmes.
- To implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing.
- To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients' health.
- As required, to provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSF.
- To prescribe medication for patients in accordance with PGDs and own scope of practice.
- To provide guidance to patients in the use of prescribed and over-the-counter medications regarding side effects and dosages.
- To request pathology services and process pathology results as required.
- To provide chronic disease clinics, delivering patient care, referring patients to secondary/specialist care as required.
- To ensure the appropriate maintenance and use of chronic disease registers.
- To participate in well woman & well man clinics.
About us:
The Birchwood and Sollershott Surgery is one practice based over two sites. We are an accredited education practice, which supports ST2/ST3s. With a patient list of approx 18,500 and growing, we are a busy and lively practice. We operate a Total Triage model of working, with a clinical triage team supporting the triage for patient enquiries. Our GPs have 15-minutes per patient appointment, with admin and catch up slots during the day. Our multi-disciplinary clinical team is made up of GPs, Paramedic, ANPs, prescribing nurses, Practice Nurses and Health Care Assistants. We operate in a supportive PCN area, with support from Clinical Pharmacists, Pharmacy Technicians, First Contact Physio, Midwifery, Phlebotomy, Mental Health Nurse, Social Prescribers and Enhanced Access Service.
Person Specification:
- Registered nurse (Nursing and Midwifery Council).
- Experience working in a primary care environment.
- Experience of chronic disease management.
- Experience of working as a practice nurse.
- Experience of infection prevention and control measures.
- Wound care / removal of sutures and staples.
- ECGs.
- Venepuncture.
- Understand the requirement for PGDs and associated policy.
- Ability to record accurate notes.
