We are seeking a dedicated professional Care Coordinator with Senior Care experience to lead and manage our team of Carers. Please note we are not offering sponsorship for this position. Duties will include:
- Statutory Responsibilities and Care Duties
- Ensuring the physical, social and emotional needs of our residents are met to the highest standards.
- Assist and participate in all aspects of personal care for all residents, becoming familiar with all needs and personalities.
- Setup and monitor programmes for residents care needs (food & fluid, incontinence etc.).
- Manage Resident admissions and ensure they are welcomed into the Home.
- Assist the management in reviews of residents care plans.
- Monitor quality control and ensure high standards are practised at all times.
- Provide Deputy Management cover for the Home as required and during out of hours.
- Ensure that all aspects of Health and Safety are adhered to.
- Assist the management with special functions as required.
- Administration duties
- Ensuring that all Policies and Procedures for the ordering, receipt, recording, storage, handling, administration and disposal of medicines are adhered to.
- Perform a monthly med audit and assist with daily med audit performed by Senior staff.
- Ensure the Doctor's weekly round and District Nurse visits are supervised and utilised for maximum benefit.
- Facilitate regular assessments of residents and document appropriately.
- Manage and monitor CHATs visits, social worker reviews and community nurse rounds.
- Staff management responsibilities
- Ensuring that Care staff are deployed and conducting their daily routines efficiently to the required standard and provide cover when required.
- Perform daily handover, set roles and allocations for staff on a daily basis.
- Manage staff in terms of shift coverage and deployment for sickness and absences, reporting to the management with concerns, issues and requirements.
- Provide support/supervision for the performance management of staff and to be responsible for effective team working.
- Assist management with the supervision/appraisal of staff and the probationary review of new staff.
- Ensure doorbell and telephone are answered efficiently and in a professional manner.
- Monitor daily room checks, ensuring high standards of environment are maintained and report all maintenance issues to management.
- Participate in appropriate staff training activities.
- Assist and participate in recruitment and induction process for new staff as required.
- Chair Senior meetings to ensure communication between management and care staff.
- Liaison
- Liaison with Pharmacist as necessary.
- Meeting and update with Manager each day.
- Liaise with and monitor Chiropody, Hairdressing, Dentist, Optician, Hospital Visits and all other external agencies appropriate to providing a high-quality service.
- Book, manage and chase up appointments for residents (blood pressure, transport etc.).
- Liaise with relatives about current conditions and concerns.
- Attend all meetings as required.
Requirements:
- Minimum NVQ3 in Healthcare or equivalent qualification.
- Proven experience of leading a care team.
- Passion for elderly / dementia care, with a hands-on approach.
- Competent with basic computer software, such as Word, web based systems etc.
