You will delivertrainingon the service, pumps and pen devices to enable the patient or carer to safely self-medicate

Monitor the patients response to treatment

Provide ongoing support to patient via telephone calls, video calls orhome visits

You will manage your own diary and schedule pre-assessment,titration,and post-assessment visits

Liaise with hospital HCPs regarding clinical changes with the patient
