What the job involves
- Provide general medical care to registered patients, including assessment, diagnosis, treatment and ongoing management of acute and chronic conditions
- Conduct face-to-face, telephone and online consultations according to clinical need and practice procedures
- Prescribe safely in line with current guidance and practice policies
- Refer patients to secondary care and other services when clinically indicated
- Deliver preventative care, health promotion, screening and monitoring of long-term conditions
- Identify and manage urgent and emergency presentations appropriately
- Keep accurate, timely and confidential records on the practice clinical system
- Work with the wider multidisciplinary team to support patients with complex needs
- Undertake duty doctor sessions
- Carry out minimal home visiting, with a paramedic team available
- Act as clinical lead for an aligned care home
