The post holder will provide clinical expertise and operational management to the Virtual Wards (Hospital @Home) model within Birmingham Community Healthcare NHS Foundation Trust.
The role involves working across city-wide settings, collaborating with Secondary and Primary Care, and facilitating early discharge while preventing avoidable admissions.
Shifts are 08:00–18:00, 37.5 hours per week, fixed-term for 12 months; closing date is 16/11/2026.
