We are looking for Nurse or Paramedic Streamer (Adults and Children) to work in Queen Mary Sidcup Urgent Care and Out of Hours Service and Erith Hospital.
Main duties of the job:
- Delivery of safe and effective nursing care to patients.
- Involved in the initial assessment of ambulatory adult cases and responsible for care delivered in streaming / triage process.
- Autonomous decisions on directing patients to the correct clinical area of service.
- Professionally and legally accountable for all aspects of work.
- Critical thinking and skills in clinical decision-making.
- Support patients with streaming and redirection tool.
- Identify patients with red flag symptoms and elevate.
- Assess the physical and psychosocial needs and develop a plan of care with the patient.
- Observe, record and report all changes.
- Produce accurate, contemporaneous and complete records.
- Deliver care according to the National Framework 2022, NICE guidelines and evidence-based care.
- Support colleagues with minor injuries / illness caseload.

BHNC will make a positive impact on medical services in Bexley, by working closely with the CCG, local NHS trusts, local providers and patient groups, to improve the delivery of healthcare to the local population.
Job responsibilities:
- Ensure the delivery of safe and effective nursing care to the Urgent Care patients.
- Involved in dealing with the initial assessment of all (adults and children) ambulatory adult cases arriving at the Urgent Treatment centre.
- Make autonomous decisions on directing patients to the correct clinical area of service.
- Be professionally and legally accountable for all aspects of their professional and clinical work within agreed national, local and professional standards and guidelines.
- Responsible for the care delivered during the streaming / triage process.
- Demonstrate critical thinking and skills in clinical decision-making.
- Work collaboratively with the whole Urgent Care team to meet the needs of patients.
- Support patients using the streaming and redirection tool at the first point of contact.
- Identify patients who present with red flag symptoms.
- Recognise and work within own competence and professional code of conduct.
- Provide medication administration under Patient Group Directions.
- Use enhanced skills and knowledge to assess the physical and psychosocial needs and develop a comprehensive plan of care with the patient.
- Observe, record and report all changes to a senior clinician in a timely manner.
- Produce accurate, contemporaneous and complete records of patient consultation.
- Prioritise, organise and manage own workload.
- Deliver care according to the National Framework 2022, NICE guidelines and evidence-based care.
- Support Emergency Clinical Practitioners, Advanced Clinical Practitioners and Urgent Care GPs with caseload in minor injuries/illness.
- Provide health education and promote healthy lifestyle for patients and carers.
- Promote safe systems of work and identify areas for improvement.
- Ensure resources are used efficiently and effectively.

Person Specification:
- Registered Nurse or Paramedic.
