Bexley Health Neighbourhood Care (BHNC) is a GP Federation supporting 20 GP practices across four Primary Care Networks (PCNs) in Bexley, serving a population of over 246,000 patients. The Assistant PCN Business Manager will play a key role in supporting delivery of the Bexley GP Premium and support with other PCN associated KPIs.
Main duties of the job
- Lead on GP Premium monitoring, data oversight, practice engagement and performance improvement, working closely with the PCN Business Manager to support practices in maximising delivery, quality and financial achievement.
- Provide operational leadership across GP Premium indicators, ensuring robust monitoring, proactive engagement with practices, and alignment with borough, SEL and national strategic priorities.
- Act as a key link between practices, PCNs, BHNC, and system partners.

Job responsibilities
- Location: Bexley Office based (with travel required as necessary across Bexley)
- Key relationships include but are not limited to: PCN Business Manager, BHNC Board, BHNC Chief Executive Officer, PCN Clinical Directors, PCN Governing Body Members, BHNC Senior Management, Practice Managers and GP Leads, Data, Finance and Quality Leads, ICB and system partners, VCSE and community partners (where relevant).
Key Responsibilities - GP Premium Leadership and Delivery
- Lead on the operational delivery and monitoring of the Bexley GP Premium across all PCNs and member practices.
- Act as the primary point of contact for practices regarding GP Premium requirements, performance, and evidence submission.
- Support practices to understand GP Premium indicators, timelines, and expectations, providing clear guidance and advice.
- Work with the PCN Business Manager to ensure GP Premium delivery aligns with wider PCN DES, IIF, Capacity and Access, and neighbourhood priorities.
Day-to-Day Support to PCN Business Manager and PCNs
- Provide day-to-day operational support to the PCN Business Manager across GP Premium, PCN DES, IIF, Capacity and Access and neighbourhood priorities.
- Support the coordination of PCN activity, including meeting preparation, action tracking, data collation and follow-up with practices.
- Manage routine correspondence and queries from practices, resolving issues where appropriate and escalating complex matters.
- Support the PCN Business Manager with data analysis, reporting and assurance returns, using EMIS, Ardens, ICB dashboards and CESEL data.
- Assist with the coordination of improvement activity, practice support visits and follow-up actions.
Performance Monitoring and Data Oversight
- Develop and maintain robust monitoring frameworks to track GP Premium performance at practice, PCN and borough level.
- Use EMIS, Ardens, ICB dashboards, CESEL data and other intelligence tools to monitor progress against indicators.
- Identify risks, variation and underperformance early, and support practices with targeted improvement actions.
- Produce regular performance reports and exception summaries for the PCN Business Manager, Clinical Directors and BHNC leadership.
Practice Engagement and Improvement Support
- Build strong, trusted relationships with Practice Managers, clinicians and operational teams across all PCNs.
- Proactively engage practices to support delivery of GP Premium activity, including holistic LTC reviews, frailty, cancer, screening, and other priority areas.
- Support practices to implement improvement plans where delivery is off track, sharing best practice and learning across the borough.
- Facilitate practice-level and PCN-level meetings, workshops and engagement sessions focused on GP Premium delivery and improvement.
Strategic Alignment and Transformation Support
- Ensure GP Premium delivery is aligned with neighbourhood health and integrated care priorities, long-term condition management and prevention, and health inequalities reduction by reducing variation.
- Support the PCN Business Manager in translating strategic objectives into practical, deliverable actions for practices.
- Contribute to the development of PCN and BHNC plans, reports and assurance returns relating to GP Premium and primary care performance.
Governance, Assurance and Reporting
- Support assurance processes by ensuring appropriate evidence is collated, quality checked and submitted in line with requirements.
- Escalate delivery risks, data quality issues or system barriers to the PCN Business Manager in a timely manner.
- Develop assurance report with migrations and action plans for PCNs.
Partnership Working
- Work with finance team to ensure accurate and timely payments.
- Provide PCNs / practices finance reports as required.
- Maintain accurate and timely financial accounts.
- Work collaboratively with BHNC colleagues, PCNs, the ICB and other system partners to support joined-up delivery.
- Support alignment between GP Premium activity and related initiatives such as neighbourhood teams, care coordination, and social prescribing.
