Bexley Health Neighbourhood Care (BHNC) is a GP Federation supporting 21 Bexley Practices/4 PCNs, Erith Hospital and Queen Marys Urgent Treatment Centre. The post holder will be responsible for the assessment of care needs and the development, implementation and evaluation of programmes of care within the Urgent Treatment Centre.
Deliver excellent patient care through continuous review of evidence-based clinical standards for patients presenting with undifferentiated diagnosis with illness and or injury. Contribute to the quality, monitoring, evaluation and the development of the service.
Key Responsibilities- Use holistic patient assessment skills of adults and children so that clinical decision making will ensure prompt and appropriate action in the best interest of the patients/families' well-being.
- Undertake assessment of health and presenting complaint, diagnosing, treating, giving advice and discharging or referring patients to other health professionals including the GP and speciality on call teams according to protocols.
- Request and interpret X-rays and emergency blood tests if appropriate.
- Act as an effective role model, through the application of skills to patient care acting as advisor to the wider MDT.
- Prescribe medications under own professional scope of practice and within the formulary acknowledging own professional boundaries.
- Keep accurate and immediate records of every enquiry including assessment and advice regarding patient care using both Adastra, Cerner and EMIS.
- Encourage patients to carry out self care as appropriate through effective communication and health care advice.
- Be responsible and accountable for all the patients on own caseload, seeking guidance from the Clinical Lead GP when clinically appropriate.
- Maintain own professional development, identify training needs and attend relevant courses necessary to ensure best practice.
- Keep up to date with current developments in patient care and evidence based practice.
- Work within all policies, protocols and guidelines of the UTC and BHNC.
- Ensure that confidentiality is maintained at all times including the storage of records and the transfer of verbal and written information.
- Work within multidisciplinary care teams promoting good communication with the Trust and hospital staff, statutory and voluntary agencies.
- Ensure that patients and carers who face cultural or language barriers are helped to understand and make use of the service available.
- Identify safeguarding issues patients may have and act on the information.
- Contribute to the recruitment and selection process, support other members of the health care team.
- Take part in the day to day management of the team at the UTC, maintain effective communication with stakeholders and other outside agencies.
- Represent the organisation positively and professionally, behaving as an ambassador for BHNC by displaying the organisation's values and levels of expected professionalism at all times.
