Clinical Pharmacist in Dartford

An exciting opportunity to recruit an experienced Clinical Pharmacist, with CPPE & IP or willing to work towards an approved 18 month CPPE training pathway and independent prescribing course, to join a multi-disciplinary team at Crayford Town Surgery, DA1 4FN. The role involves working as part of a multi-disciplinary team (MDT) to drive forward the PCN vision of creating a wider Community Service, enhancing existing and innovatively developing new care models in collaboration. It engages groups of practices to improve the quality of care and operational efficiencies across the local healthcare system, and leads the delivery of network priorities and targets with a practice-based focus.

Within professional boundaries, the role supports development, promotion and implementation of high-quality evidence-based research to enable cost-effective use of medicines while minimising clinical, legal, or financial risks.

Job responsibilities

• Manage own caseload of patients and deliver services with patient-first care for common/minor/self-limiting ailments within scope of practice and competence.
• Provide patient-facing clinics for questions about medicines in the practice, especially for long-term conditions, signpost to community pharmacy, and refer to GPs or other healthcare professionals where appropriate.
• Provide follow-up to monitor effects of changes, including virtual advice and responses to medicine-related enquiries from GPs, network staff, other healthcare teams, and patients.
• Use structured medication reviews to support patients in taking medicines, reduce waste and promote self-care.
• Liaise with BHNC colleagues on prescribing matters to ensure consistency of patient care and benefit.
• Refer patients to appropriate healthcare professionals for the appropriate level of care within suitable timeframes.
• Collaborate with hospital colleagues to manage high-risk patients post-discharge and reconcile medicines after discharge.
• Produce accurate, contemporaneous records of patient consultations in line with legislation and policy.

Financial & Resources

• Accountable for adherence to the budget, identifying efficiencies and communicating cost implications of non-compliance to senior managers.
• Manage resources and budget within delegation; develop cost-benefit analyses where required.
• Develop and maintain effective working relationships within and outside the PCN, including community pharmacists and other local services.
• Liaise with BHNC, STP/ICS Pharmacists, Pharmacy Technicians, and Heads of Medicines Management/Optimisation to ensure consistency and benefit in prescribing.
• Foster links across services and explore collaborative working opportunities for patient benefit.

Research & Development

• Contribute to BHNC and PCN visions and business objectives; advise on prescribing for projects and change initiatives.
• Review ongoing need for medicines and support medicines optimisation; assist public health campaigns with specialist knowledge.
• Undertake clinical medication reviews and work with care home staff to improve safety of medicines ordering and administration.
• Refer patients to multidisciplinary case conferences to ensure medicines optimisation.
• Review medicines related to high-risk admissions and implement changes to reduce prescribing for these groups.
• Participate in professional clinical networks and ensure access to appropriate clinical supervision.

Policy & Service Development

• Provide pharmaceutical advice for new services with medicinal components; contribute to treatment pathways and patient information leaflets.
• Develop and implement a practice repeat prescribing policy, manage repeat prescription requests, and ensure monitoring tests where required.
• Conduct clinical audits of prescribing; implement changes with the practice team and support prescribing safety quality improvement.
• Implement MHRA alerts and national guidance; monitor prescribing against local health economy indicators and maintain practice formulary.
• Audit compliance with NICE technology assessments and NSF; provide newsletters on prescribing messages.
• Work with practice teams to ensure CQC standards compliance where medicines are involved.
• Attend local, regional, and national meetings relevant to the GP pathway for this role, supported by CPPE.

Qualifications

• CPPE General Practice Pharmacist Training Pathway or equivalent, and willing to work towards with exemption agreed by CPPE;
• GPhC registration;
• MPharm or equivalent;
• Safeguarding and mandatory training;
• Independent prescriber course.

Experience

• Minimum of 2 years as a pharmacist in a practice setting;
• Experience of partnership working and building cross-organisational relationships;
• Understanding of common acute and long-term conditions in general practice;
• Appreciation of GP/practice prescribing concepts.

• Ability to analyse complex data, identify and resolve risk management issues, prioritise problems, and refer when needed;
• Interpret medicines data for decision making;
