An exciting opportunity to recruit an experienced Clinical Pharmacist, with CPPE & IP or willing to work towards an approved 18 month training pathway (CPPE) and independent prescribing course, to join our multi-disciplinary team at Lakeside Medical Centre, Thamesmead SE2 9LH. Working as part of a multi-disciplinary team (MDT), the job holder will take a lead in driving forward the PCN vision of creating a new wider Community Service, enhancing existing and innovatively developing new care models in a collaborative way. This role plays a pivotal part in engaging groups of practices to come together and improve the quality of care and operational efficiencies across the local healthcare system, motivating passion to deliver an excellent service within general practice, whilst leading and tailoring the delivery of network priorities and targets with a practice based focus.

Within their professional boundaries, support the development, promotion and implementation of high quality evidence based research that enables the cost effective use of medicines in a way that maximises the benefits of medicines to patients whilst minimising clinical, legal, or financial risks.

Main duties of the job

  • Prescribers in this patient-facing role will proactively shape structured medication reviews, ensuring that medicines management processes within the practice are optimised to achieve effective, safe, and patient-centred prescribing.
  • They will lead the way in supporting patients to manage their medicines where necessary, reducing waste and promoting enhanced self-care.
  • Undertaking structured training if the job holder has not had experience in primary care.
  • Offering peer support and facilitating shared learning with other pharmacists in local practices across the networks to support development and implement agreed prescribing and pharmaceutical activities or development programmes.

Job responsibilities

  • Manage own caseload of patients and deliver services putting the patient first with common/minor/self-limiting ailments within scope of practice and competence.
  • Provide patient-facing clinics for medicines questions and concerns, especially for long-term conditions; signpost to community pharmacy and refer to GPs or other professionals where appropriate.
  • Provide follow-up to monitor effects of changes, including virtual advice and answering medicine-related enquiries from GPs, network staff, other healthcare teams and patients.
  • Use structured medication reviews to support patients in taking their medicines, reduce waste and promote self-care.
  • Liaise with BHNC colleagues on prescribing matters to ensure consistency of care and benefit.
  • Refer patients to appropriate healthcare professionals and ensure timely care.
  • Collaborate with hospital colleagues to manage high-risk patients post-discharge and reconcile medicines after discharge.
  • Produce accurate, contemporaneous records of patient consultations in line with legislation, policies, standards and procedures.

Financial & Resources

  • Accountable for adhering to the budget, ensuring best value, identifying efficiencies, and informing senior managers of cost implications of non-compliance.
  • Manage resources and develop cost-benefit analyses where required.
  • Build relationships within and outside the PCN and with community pharmacists and other providers.

Research & Development

  • Contribute to the BHNC and PCN visions, aims and objectives; advise on prescribing in projects and change initiatives; monitor ongoing need for medicines and support medicines optimisation.
  • Support public health campaigns with specialist knowledge; undertake clinical medication reviews and refer to multidisciplinary teams; review medicines used in high-risk settings to reduce inappropriate prescribing.
  • Participate in clinical supervision with defined supervision expectations and ratios.

Policy & Service Development

  • Provide pharmaceutical advice for new services with medicinal components; implement repeat prescribing policies and monitoring; conduct clinical audits and respond to MHRA alerts and national guidance.
  • Monitor practice prescribing against local health economy guidance and NICE/NHS frameworks; maintain formulary; communicate prescribing messages via newsletters or bulletins.
  • Assist practices in meeting CQC standards where medicines are involved; attend relevant meetings as defined by CPPE GP pathway.

Qualifications

  • Essential: General Practice Pharmacist Training Pathway from CPPE, or equivalent, or willing to work towards with CPPE exemption agreed. Mandatory General Pharmaceutical Council registration; MPharm or equivalent; safeguarding and mandatory training; Basic Life Support. Royal Pharmaceutical Society membership and completed Independent Prescriber course.
  • Experience: Minimum of 2 years of experience as a pharmacist, demonstrated within a practice portfolio. Experience of partnership/collaborative working and building relationships across organisations. Experience and awareness of common acute and long-term conditions in general practice. Understanding of GP practices and primary care prescribing concepts and rational prescribing.
  • Desirable: In-depth therapeutic and clinical knowledge and understanding of evidence-based healthcare.

  • Able to obtain and analyse complex data; identify and resolve risk management issues; recognise priorities and refer where needed.
  • Analyze, interpret and present medicines data; excellent interpersonal, influencing and negotiation skills; communicate complex information clearly to various audiences.
  • Build relationships with stakeholders and engage audiences to achieve agreed actions.
