This is a fantastic opportunity to join our team at Brooklands Medical Practice. You will join an established pharmacy team, supporting with all aspects of medicines optimisation across our practices. You will work closely with our large clinical team including GPs, Nurses, Physiotherapists, Mental Health Practitioners, Physicians Associates and Wellbeing Team.

Please note, the successful candidate will initially be employed by Brooklands Medical Practice pending transition into BHM.

Main duties of the job:

  • Support with prescription related requests and queries from patients and monitor medicines usage in line with long term condition management.
  • Review clinical letters and hospital discharges and action appropriate medication changes and where necessary support patients with optimisation of their management.
  • Undertake clinical medication reviews with particular focus on those with additional need such as polypharmacy, long-term conditions, multi-morbidity, frailty, or multi-factorial need.
  • Work with the organisations prescribing lead and the wider ICB medicines management team to ensure evidence based and cost-effective prescribing in line with the latest guidance and safety alerts.
  • Be an experienced pharmacist with a knowledge and commitment to working in primary care.
  • Understand the needs of patients from deprived and diverse backgrounds.

We operate three GP practices: The Robert Darbishire Practice in Rusholme Health Centre (which is also head office) and which has a branch at The University of Manchester, Brooklands Medical Practice in Wythenshawe and The Whitswood Practice in Alexandra Park Health Centre. In total we have over 44,000 patients. Our patients are very diverse and include established communities as well as new communities from all over the world. We have good links with the University and a sizeable student population. Our staff team of about 120 people has a great mix of skills and experience. We have exciting plans to expand this team even more, to help us further our mission to improve lives.

The post is part-time (30 hours) and will be based primarily at Brooklands Medical Practice. The usual working hours are to be arranged but will be between 08:30 and 18:30 Monday to Friday. Salary will be in the range of £47,810 to £54,710 depending on experience. We offer membership of the NHS Pension Scheme and up to 7.6 weeks holiday which includes Bank Holidays, depending on NHS length of service.

Person Specification:

  • Pharmacist with a minimum of two years post-registration clinical experience.
  • Experience of working in a deprived and ethnically diverse population.
  • Recent experience working as a Pharmacist in primary care.
  • Registered Pharmacist.
  • Membership of General Pharmaceutical Council / Royal Pharmaceutical Society.
  • Non-medical Prescriber.
  • Completion of CPPE PCPEP.
  • Post-graduate diploma in clinical pharmacy or equivalent.

Personal Skills and Abilities:

  • Understanding of general practice.
  • Understanding of primary care prescribing.
  • Expert knowledge and application of the principles of evidenced-based healthcare.
  • Excellent communication skills the ability to communicate clearly and effectively with a wide range of individuals both verbally and in writing.
  • The ability to communicate and interact with the public in a discreet and sensitive manner, recognising their needs for alternative methods and styles of communication.
  • Good organisational skills.
  • A strong commitment to providing a high standard of care that is responsive and flexible in its approach.
  • Willingness and aptitude to be involved in teaching.
  • A commitment to professional and personal development, with evidence of recent study and learning.

Other:

  • Good IT and keyboard skills.
  • A full, valid UK driving licence with business car insurance and access to a car on a daily basis.
