An exciting opportunity has arisen for a Team Manager with the Community Resources Team in Wrexham.

The purpose of this post is to provide support to the Community Resource Team in the development, implementation and co-ordination of procedures and caseloads.

Main duties of the job

The successful candidate will lead and manage the service to ensure a seamless service is provided to enable patients to be managed safely and appropriately in the home environment. You will coordinate and supervise the team to ensure a patient centered service.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Qualifications

  • NMC Registered Nurse
  • Post Graduate Specialist Practitioner Degree
  • Evidence of ongoing education & professional development
  • Mentorship & assessment training

Other Relevant Requirements

  • Ability to travel
  • Flexibility, including lone working
  • Demonstrate timely travel arrangements between patients’ homes, clinics & surgeries
