Applications are welcomed from enthusiastic practitioners who want to make a difference in primary care service provision. The successful candidate will support and facilitate the organization of clinical activity, assist in the delivery of group and individual work, and continue to support and deliver clinical activity as required. The post holder must work independently, liaise with third sector agencies and carers, and maintain excellent multidisciplinary working links with GP practices.
Responsibilities
- Provide support and allocate time to an assigned group of service users, promoting recovery and enabling them to return to or maintain community living.
- Support service users to meet identified needs and outcomes, respecting their values and aspirations, and help them access community resources such as housing, benefits, leisure, education, and employment.
Qualifications and Experience
- Certificate in Community Mental Health Care / NVQ Level 3 in care or equivalent.
- Good literacy and numerical skills.
- Aware of issues affecting mental health service users.
- Knowledge of approaches to working with mental health service users.
- Knowledge of housing and welfare benefits, housing issues, and related employment and educational issues.
- Ability to assess and evaluate individual needs to enable service users to make informed decisions.
- Ability to analyse complex information concerning mental health, housing, finance issues.
Desirable Criteria
- Qualifications in Health, Social and Care.
- Administrative and organisational skills.
Language Requirements
The ability to speak Welsh is desirable. Welsh and/or English speakers are equally welcome. Applications may be submitted in Welsh and will not be treated less favourably than an English application.
