We have an exciting opportunity in Wrexham CAMHS for a Pathway Lead for Transitions and Secondary Care. Our vision is to continue with the development of a more streamlined approach to practice across pathways. Pathway leads will ensure that a high quality, timely, accessible and evidence-based provision is available to all children and young people who need it.

Responsibilities

  • The Pathway Lead for Transitions and Secondary Care in CAMHS will have a solid understanding of the Mental Health Measure Wales 2010.
  • You will have experience of managing a caseload of young people with a range of mental health needs and supporting young people through their transition to adult mental health services.
  • You will also lead on the monitoring and delivery of this clinical pathway and provide leadership support to pathways that have interdependencies with transitions and secondary care.
  • In addition, you will have experience of being responsible for providing evidence-based assessments and therapeutic treatment for these individuals alongside supervising CAMHS Practitioners with casework.

Qualifications

  • Applicants should have a relevant professional and registered qualification relating to Health or Social Care e.g. in social work, mental health or learning disability nursing, or Occupational Therapy.
  • A high level of competence, knowledge and skills (underpinned by knowledge of effective risk assessment and relevant evidence-based practice) which is crucial to this role.
  • The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

  • The ability to use initiative when dealing with unexpected situations is also an advantage.
  • Applicants should be able to demonstrate evidence of their clinical understanding and decision-making skills; be a committed team player with the ability to prioritise their own workload, and work well under pressure.
  • Good IT skills are essential with confidence in Outlook, Word and Excel.
