Job Overview

We will only accept applications from staff currently employed by Betsi Cadwaladr University Health Board. An exciting opportunity has arisen for an enthusiastic and motivated Band 6 Deputy Clinical Flow Manager to join the multi-disciplinary Home First Team based in Wrexham. The post is 37.5 hours covering 7 days a week between 8am and 8pm.

Responsibilities

  • Deputise for the Clinical Flow Manager for Community Services and support the Home First Team Leader.
  • Responsible for ongoing screening of patients to facilitate safe and timely discharge from hospital via the Discharge to Assess model of care.
  • Act as a link between acute and community services and work alongside the Community Resource Team in Wrexham and Flintshire.
  • Promote the Home First ethos through therapeutic interventions in the patient’s own environment.
  • Be actively involved in the future development of the service.

Qualifications and Knowledge (Essential)

  • First Level Nurse registered with the NMC.
  • Evidence of a degree level qualification or working towards this within an agreed time frame.
  • Evidence of CPD.
  • Holds or is actively working towards a relevant Post-graduate Diploma, to be completed within an agreed timeframe.

Desirable Qualifications

  • Management qualification and/or management experience.
  • Mentor/supervisor/assessor role.

Experience (Essential)

  • Significant experience working within the relevant clinical speciality or nursing role.
  • Ability to demonstrate application of improvement methodology.

Desirable Experience

  • Experience working in a multidisciplinary team setting.
  • Experience/knowledge of senior nurse role.

Aptitude and Abilities (Essential)

  • Ability to communicate and receive complex information.
  • Ability to diagnose a problem and take corrective action.
  • Ability to analyse assessment results and transfer them into relevant packages of care.
  • Ability to work autonomously as well as part of a team.

Desirable Aptitude

  • Ability to speak Welsh.
  • IT skills.
  • Ability to manage resources.
