Trainee Cardiology ANP in Wales

The Cardiology ANPs at Glan Clwyd Hospital provide a high quality, comprehensive and innovative nurse led service for patients who experience Acute Coronary Syndromes. They provide and facilitate timely care and interventions for patients experiencing Acute ST Elevation Myocardial Infarction (STEMI) or high risk Non-ST Elevation Myocardial Infarction (NSTEMI) for the whole of North Wales and complete the care with their 'Post Intervention Clinics'. The team consists of 6 ANPs (F/T and P/T) who work very closely with all members of the Cardiology multi-disciplinary team. They are a hospital based service providing inpatient care for patients presenting with an acute Cardiology condition and outpatient care for those attending the Rapid Access Chest Pain Clinic. They provide expert advice and support to other clinical colleagues, particularly the Acute Medical Consultants and Cardiologists and offer education, training and clinical expertise to Primary and Secondary care staff.

The post holder will be practising under supervision as a trainee advanced practitioner within cardiology. This will encompass the skills of assessment, examination, diagnosis and treatment within an agreed scope of practice throughout Cardiology services.

• To work as a trainee ANP coordinating Primary PCI calls.
• Identifying patients with acute coronary syndrome within the emergency quarter.
• Assist and advise allied health professionals MDT on ECGs and treatment of STEMI and ACS.
• Working closely with CCU and MDT to ensure cardiac patients receive quality care in the correct environment.
• Complete local and national audits.
• Teach junior members of staff, and other members of MDT.

• Learn to identify critically ill cardiac patients within the acute-setting including those who require PPCI.
• Learn to escort the patient throughout their PPCI pathway acting as support to the Catheter Laboratory Team.
• Work alongside DSMP & Cardiology ANP to develop confidence and competence to autonomously run RACPC with responsibility for clinical standards, competence assessment and supervision of other health professionals.
• Work alongside DSMP & Cardiology ANP to gain expert clinical knowledge to be able to autonomously advise in the management of ACS patients in all areas of the hospital including Unscheduled Care.
• Will develop a knowledge base that is such they can deliver specialist education and training to a broad range of health professionals including nurses, doctors and ambulance personnel.
• To undertake research and lead clinical audit in own speciality with organisational responsibility for compliance with relevant national audit programmes.
• Complete clinical competencies in order to support the Cardiac Catheter Laboratory in the North Wales Primary Percutaneous Cardiac Intervention Service (PPCI).
• To learn to recognise patients with changing health status and to take appropriate actions.
• Communicate highly complex and highly sensitive information with patients and families and within the healthcare team.
• Responsible for the safe use of highly complex patient monitoring and support equipment within the ED/CCU/Catheter Lab.
• To learn interpretation of laboratory and radiological results and initiate appropriate patient management.
• Maintain own clinical development by keeping abreast of new treatments and technologies within cardiology.
• To rigorously review with a supervisor all aspects of the patients plan of care and identify and address issues that may result in a sub-standard service.
• Working alongside Cardiology ANP will develop the ability to safely triage referral received patients with suspected Acute Coronary Syndrome (ACS) and make a comprehensive clinical assessment to include examination of their health care needs.
• If appropriate to transfer patients for primary PCI to Catheter lab and to CCU/ward if necessary.
• Evaluates the care given with patients and orders necessary investigations, and takes responsibility for acting upon results, providing treatment and care both individually and through referral to Cardiologists or specialists as appropriate.
• Provide expert professional advice and liaise with patients, the Emergency Department, Acute Admissions Unit, Cardiac Catheter Laboratory, Coronary Care Unit and Cardiology Ward and colleagues.

• Evidence of advanced ECG interpretation

• CCU/Cath lab competencies or willingness to undertake
• Highly developed communication skills to be applied at all levels within situations which need persuasive, motivational, negotiation, empathetic or reassurance skills.
• Evidence of involvement in: Leadership, Team building, Managing resources, Time management, Change management/Clinical Developments, Audit and research.
