The Community Mental Health Team are seeking a skilled, motivated, organised and enthusiastic Mental Health Nurse/Practitioner to work within an established integrated, multi-disciplinary team, focusing upon individuals with severe and enduring mental health problems. The successful post holder will undertake responsibility for the care coâordination and case management of a defined caseload of clients with complex and challenging needs as defined within Parts 2 and 3 of The Mental Health (Wales) Measure 2010.
Main duties:
- To be responsible for the care coâordination of a designated caseload of clients with severe and enduring mental health problems.
- To ensure that the Care and Treatment Plans are adhered to and that standards are met in line with the Mental Health Measure (Wales) 2010.
- To act as a point of contact for GPs and other interface services to offer advice, support and signâposting to appropriate agencies.
- Formulate and develop standard and complex outcomeâfocused care and treatment plans.
- Ensure that a comprehensive risk assessment is completed and documented for actual and potential risk.
