This post involves working with dietetic colleagues, medicines management and primary care to support the development, implementation and evaluation of best practice in relation to prescribed nutritional supplements across Wrexham and Flintshire. The varied post offers the opportunity to develop and broaden leadership and team working skills, through leading on project work and training, whilst still retaining direct patient contact. Quality improvement initiatives are welcomed in the department – and the clinical community department is due to start a large service improvement project in the Autumn.

The successful candidate will be part of a department of over 50 Dietitians and Dietetic Assistants, supported by an admin team. There are other Prescribing Support Dietitians within the Health Board and good relationships between the three areas mean they are able to collaborate on initiatives and on developing best practice.

Please note we are advertising another part time post if you would be interested in more hours.

Main duties of the job

To work as an integral part of the clinical community dietetics team

To be the liaison between dietetics and medicines management teams to ensure that all patients on nutritional supplements are reviewed by the most appropriate professional and the most cost effective, clinically appropriate product is prescribed. It is recognised that this part time post cannot be responsible for seeing all of the patient group, but the post holder will be the initial point of contact for medicines management staff.

To review patients who have been prescribed oral nutrition supplements, alongside dietetic colleagues

To work with dietetic colleagues to plan, organise and facilitate appropriate education and training of medicines management staff, community health and social care professionals and care home staff on nutritional risk screening, first line dietary advice to promote a food first approach, referral for specialist nutritional support, appropriate assessment of nutritional needs, treatment options and follow up reviews as per NICE guidelines.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
