The post holder will be responsible for Older Persons Services, covering GP surgeries in the North Denbighshire area. The role includes CPA reviews and ward rounds for inpatients, outpatient clinics and medical input to the memory service.
The post holder will work within the Mental Health (Wales) Measure 2010 and provide medical leadership, consultant and responsible clinician services, as well as care planning and reporting to support long-term health care commissioning.
