BCUHB in Wales is seeking a Health Care Assistant on a fixed term/secondment until 31 March 2027 to support the safe discharge of patients from hospital care to home or community settings.

You will work with the Discharge Unit to deliver compassionate, person-centred care in short and long stay areas, maintain clear records, and support patients and families through the discharge process. Welsh language ability is desirable but not required.
