To assist registered nurses/practitioners to deliver care to patients/clients, working in partnership with other health care professionals, the patient, their family and carers.

Main duties of the job:

  • Deliver patient care as documented in the nursing care plan.
  • Carry out a wide range of duties under the direction of a registered nurse for the period of the shift.
  • Support the district nursing team to care for community patients who require nursing care.
  • Travel between patients’ homes within towns and rural areas in a timely manner.

Applicants should be enthusiastic, aware of and committed to modernisation of the service, and able to work well within a team and also alone. Welsh speaking is desirable (English speakers also welcome).
