Through the teaching of patients, families and colleagues, in order to enhance clinical practice and improve patient care. To provide a consistent approach and share best practice across BCUHB.

Job Summary / Job Purpose

To provide Specialist Nursing expertise, knowledge and skills in the Clinical practice of Transplantation Nursing Care. To be an agent for change within defined clinical and organisational boundaries. To function at a higher level of practice demonstrating higher levels of decision making, planning and collaborative working, on a regional and national basis. To provide clinical teaching and education to colleagues in order to enhance clinical practice and improve patient care. Lead in evaluation, audit and research to ensure practice is evidence based in order to improve standards of care. The role focus is to provide a continuum of care from pre-transplant identification, assessment, referral through to long-term post-transplant care.

Main Duties of the Job

  • Assess, plan, implement and evaluate care and treatment using specialist clinical knowledge and skills, advising patients and relatives as appropriate.
  • Work within a multidisciplinary team providing highly complex information and practical, evidence-based care for people requiring transplantation, liaise with consultants, named nurses and the multidisciplinary team to maintain continuity of care across BCUHB, Royal Liverpool University Hospital, Regional Transplant Centre and Manchester Royal Infirmary transplant centre.

Qualifications

Essential Criteria:

  • Nationally recognised Renal qualification or equivalent experience
  • Registered Nurse

  • Teaching Qualification

  • Experience of staff & patient training.
  • Working with patients with CKD on Dialysis
  • Understanding of frameworks and policies relating to renal care

  • Managing MDTs
  • Change Management
