Responsibilities

  • Implement and monitor Continuing NHS Health Care funding (CHC) in Care Homes/domiciliary settings and NHS Funded Registered Nursing Care (FNC) in Care Homes (Nursing) in accordance with National Guidance.
  • Coordinate and scrutinise CHC applications, participate in CHC and FNC reviews, and attend Multi‑disciplinary Team (MDT) meetings within the area/department.
  • Manage the local resolution process: meet and liaise with families to discuss panel outcomes when individuals challenge eligibility decisions.
  • Handle local resolution process for Inter‑Agency Disputes: meet and liaise with appropriate Team Managers from Local Authorities regarding disputes about eligibility for CHC funding.
  • Collaborate with statutory agencies on safeguarding issues and attend POVA/Strategy meetings.
  • Work collaboratively with Discharge Liaison Nursing Teams, Community Hospitals and District Nurses to ensure safe and timely patient discharge/transfers.
  • Oversee the East Areas Complex Care Cohort of Individuals in both Domiciliary and Care Home settings with complex physical needs and disabilities.
  • Work across professional and organisational boundaries, notably with Independent Care Homes, Social Services staff and other Health Boards and Primary Care Trusts.
  • Develop and maintain relationships within statutory/voluntary/independent sectors including health care professionals, GPs, and Local Authority areas across BCUHB.
  • Promote excellent customer care throughout the Continuing Health Care process, resolve concerns locally where possible and provide/safely deliver highly complex, sensitive and contentious information to clients and families.
  • Negotiate with clients/families, delivering complex information in sensitive and emotional situations such as outcomes of CHC applications/reviews.
  • Maintain accurate case records and communicate panel decisions clearly and sensitively within agreed timescales.

Qualifications

  • Professional registration with the Nursing & Midwifery Council.
  • Degree or equivalent level of experience in health.
  • Evidence of ongoing continuing professional development.
  • Desirable: Teaching qualification.
  • Desirable: Ability to speak Welsh or willingness to achieve appropriate oral competence.

  • Evidence of leading and developing service improvement and experience of improvement measures.
  • Specialist knowledge in relevant field.
