An exciting opportunity has arisen to join our experienced team in the delivery of Cardiac Rehabilitation in Conwy and Denbighshire. We are seeking a physiotherapist who possesses experience in general medicine and has experience or interest in working across hospital and community settings such as leisure centres. The purpose of the role is to provide specialist Cardiac Rehabilitation knowledge, expertise and skills for patients and their families that promotes recovery and self-management. This requires you to work both autonomously supported by the Clinical Specialist Physiotherapist and Cardiology Exercise Physiologist and as part of a multi-disciplinary team to ensure a safe and effective pathway for cardiac patients.

Main duties of the job:

  • Working collaboratively with the Clinical Specialist Physiotherapist to deliver a high quality clinical service in the Centre Integrated Health Community.
  • The assessment and management of complex patients.
  • Developing and delivering individualised treatment programmes and working effectively with the wider multidisciplinary team.
  • To communicate with patients and other members of Multidisciplinary Team to progress rehabilitation and treatment programmes. This will include patients who may have difficulties in understanding or communicating.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Qualifications:

  • Degree or equivalent in Physiotherapy
  • HCPC registered
  • Evidence of CPD in Physiotherapy in the form of a detailed personal development portfolio.
  • Member of Clinical Specialist Interest Group
  • Membership of appropriate professional post-graduate training body
  • Clinical educators course

Skills:

  • Excellent clinical reasoning skills in physiotherapy
  • Ability to work autonomously within clinical area
  • Able to lead and participate in team working and training
  • Able to be self-motivated
  • Able to present information in a clear and logical manner
  • Proven organisational skills including prioritisation and delegation
  • Able to lead and motivate others within the team
  • Physically able to manually handle loads, including therapeutic handling, throughout most of the day
  • Competency in report writing
  • Ability to comprehend and work in accordance with the health board's Data Protection, Equal Opportunities and Health and Safety policies, in order to meet the differing needs of patients
  • Knowledge and skills of risk assessment
  • Actively involved in research and contribution to evidence-based practice
  • Able to run courses-internal/external
  • Good teaching and supervisory skills
  • Ability to create presentations

  • PDR training

Knowledge:

  • Good knowledge of complex conditions
  • Advanced knowledge of anatomy and physiology underlying complex pathology
  • Good working knowledge of all areas of clinical governance including quality, audit and risk management.
  • Understanding of the legal responsibilities of the profession
  • Good understanding of the need to collect statistics and experience of their use
  • Broad understanding of BCU policies and procedures
  • Disability awareness.

Experience:

  • Significant Post Registration Experience
  • Evidence of core rotations
  • Experience of working in an NHS environment as physiotherapist and MDT/ multi-agency working where relevant
  • Experience of contribution to CNG or working parties.
  • Experience in the relevant speciality

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
