A permanent Band 6 Caseload Holder is required for the Conwy Locality based Colwyn Bay District Nursing Team. The role reports to the Band 7 Team Leader and requires excellent communication skills and the ability to provide effective, high‑quality care.

Responsibilities

  • Provide community nursing care to patients with acute or complex needs.
  • Deliver care in a variety of settings, including patient homes, clinics and surgeries.
  • Plan, implement, evaluate and delegate care appropriately.
  • Supervise, tutor and assess registered and non‑registered staff.
  • Participate in audit, review and continuous quality improvement activities.
  • Maintain accurate professional records and adhere to data protection and confidentiality.

Essential Criteria

  • Registered with the NMC as a first level nurse.
  • Working towards or holding a Community Specialist Practitioner qualification.
  • Registered nurse prescriber (or working towards).
  • Experience as a Band 5 community nurse or equivalent.
  • Evidence of ongoing education and professional development.
  • Excellent verbal and written communication skills.
  • Commitment to continuous professional development.
  • Organisational and analytical skills.
  • Ability to review and manage patient risk, health and safety.
  • Knowledge of consent, data protection and patient confidentiality.
  • Willingness to travel between patient homes, clinics and surgeries.
  • Demonstrated ability to work collaboratively with the primary health‑care team and other agencies.
  • IT skills.
  • Experience undertaking audit (desirable).

Desirable Criteria

  • Management qualification such as ILM or RCN Leadership.
  • Experience in a link nurse role.
  • Recognition or qualification within chronic conditions management.
  • Awareness of the wider national health agenda.
  • Welsh language ability.
