Then come and join our District Nursing Service which is dynamic, proactive and progressive with a commitment to making a difference for our patients. We believe in developmental and training opportunities for our staff and therefore are currently seeking to recruit highly motivated, passionate Band 5 Community Nurses who are keen to develop and work towards one of the following roles:

  • Band 6 District Nurse by undertaking the District Nurse Qualification.
  • Chronic Condition Case Manager by undertaking disease specific training to provide evidence based care to those individuals with highly complex needs.
  • Nurse Practitioners by undertaking a modular approach to attain the clinical skills required.

Main duties of the job include having excellent communication skills with an ability to provide clinically effective care within the District Nurse Team. The Central Division of Betsi Cadwaladr University Health Board delivers District Nursing Services across the Counties of Conwy and Denbighshire, working in an integrated way with partner organisations to deliver high quality evidence based care in order to maintain patients in their own homes and communities, averting hospital admissions, empowering the patient towards self management and implementing seamless patient centred care and treatment.

You must be able to travel between patients' homes both within towns and rural areas in a timely manner. This post is rotational over 24 hours. The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
