Job Overview
This post is fixed term/secondment for 6 months due to needs of the service. If you are interested in applying for the secondment position, you must obtain permission from your current line manager prior to applying for this post.
We are recruiting for a full time Band 6 CPN to join our team on a secondment, working within the Flintshire Older Persons Community Mental Health Team.
Main duties of the job
As Care Coordinator, you will hold a caseload of patients with organic and functional mental health needs, travelling to see them in their own homes, in care homes and in-patient settings. If you are an experienced Mental Health Nurse with passion and enthusiasm for working with older adults, we would love to hear from you. The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
