This secondment is for 9 months to meet service needs. Only applicants employed by Betsi Cadwaladr University Health Board may apply. You must obtain permission from your current line manager before applying.

The role involves managing a team of Nurse Reviewers who visit patients in care homes and patients' own homes to review and assess their needs in relation to NHS Funding, in accordance with national guidance. The post holder will work closely with discharge teams across hospital sites, nursing homes, social services departments, and other professionals within the NHS, including contract teams to manage and coordinate CHC applications.

Main Duties

The post holder is responsible for providing management and clinical leadership to teams of professionally qualified nurses in the day‑to‑day delivery of Continuing NHS Health Care (CHC) and associated processes. They ensure the quality and safety of patient care in care homes within the independent sector, and are responsible for CHC services provided by the area, including assessment, applications for funding, ratification of eligibility and care packages, reviews and dispute management. The post holder takes overall responsibility for the day‑to‑day management, implementation and review of the National Framework for Continuing NHS Health Care and NHS Funded Registered Nursing Care (FNC) on behalf of the area, ensuring robust systems to establish patient entitlement to continuing NHS health care and NHS funded registered nursing care. The role involves communication with other areas, the corporate CHC team, the independent sector, local authorities, Welsh Health Legal Services, independent solicitors and the Welsh government as required. The post holder will work across 7 days to cover the Matron of the day on a rota basis. Welsh language ability is desirable but applicants who speak English or Welsh are equally welcome.

  • Ensure quality and safety of patient care in independent sector care homes.
  • Implement and review the National Framework for Continuing NHS Health Care and NHS Funded Registered Nursing Care.
  • Communicate with area teams, corporate CHC team, independent sector, local authorities, Welsh Health Legal Services, independent solicitors and Welsh Government.
  • Cover the Matron of the day on a rota basis across 7 days.
Qualifications
  • Professional registration with the Nursing and Midwifery Council.
  • Evidence of continuing competence to practice.
  • Experience Clinical background.
  • Budget and staff management experience.
  • Evidence of innovative work in previous roles.
Abilities
  • Demonstrate effective partnership working.
  • Motivate and manage staff effectively.
  • Ability to speak Welsh.
