Overview

The Community Mental Health Team are seeking a skilled, motivated, organised and enthusiastic Mental Health Nurse to work within an established integrated, multi-disciplinary team, focusing upon individuals with severe and enduring mental health problems. The successful post holder will undertake responsibility for the care co-ordination and case management of a defined caseload of clients with complex and challenging needs as defined within Parts 2 and 3 of The Mental Health (Wales) Measure 2010.

Responsibilities

  • Be responsible for the care co-ordination of a designated caseload of clients with severe and enduring mental health problems.
  • Ensure that the Care and Treatment Plans are adhered to and that standards are met in line with the Mental Health Measure (Wales) 2010.
  • Act as a point of contact for GPs and other interface services to offer advice, support and signposting to appropriate agencies.
  • Formulate and develop standard and complex outcome-focused care and treatment plans.
  • Ensure that a comprehensive risk assessment is completed and documented for actual and potential risk.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
