Responsibilities

  • Accurately assess residents' health status on admission and on an ongoing basis.
  • Communicate significant changes in condition to the appropriate interdisciplinary team (IDT) member.
  • Accurately document all assessments.
  • Develop dynamic care plans based on identified needs through an interdisciplinary approach (physicians, residents, families, and other IDT members).
  • Review and revise the care plan as resident needs, problems, and goals change; communicate the plan to IDT members, residents, and families.
  • Document the care plan on admission, cognition change, or regulatory care planning requirements.
  • Identify nursing interventions related to the plan of care, including nursing orders, physician orders, and residents'/families' educational and environmental needs.
  • Deliver interventions such as medication administration, treatments, and IV fluid administrations safely and timely; document interventions accurately.
  • Use critical thinking when addressing changes in condition; notify the physician promptly using SBARR as the communication tool.
  • Maintain composure during crises (e.g., family dissatisfaction, staff disputes, inclement weather) in a calm and professional manner.
  • Monitor care plan delivery and actively address issues through the QAPI process for immediate problem resolution.
  • Remain knowledgeable about resident status to quickly identify behavioral changes or untoward signs or symptoms.
  • Assist in answering call lights and provide resident care such as ADLs.
  • Complete a minimum of twelve hours of offerings related to nursing.
  • Uphold commitment to current best nursing practices through Bethesda policies and self-study; may be assigned other duties as appropriate.

Qualifications

  • Education from an accredited school of nursing that results in becoming a registered nurse.
  • Current RN license in the state you will be working.
  • CPR/AED certification required, or willing to obtain within six months of employment.
  • Long-term care experience preferred.
  • Must possess critical thinking skills.
  • Excellent communication skills to interact verbally with patients, visitors, and staff.
  • Excellent time management and organizational skills.
  • Ability to stand and walk for long periods of time.
  • Ability to lift heavy objects using proper lifting techniques and with assistance as necessary.
  • Ability to transfer and move patients weighing 200+ lbs with proper lifting techniques and assistance when necessary.
  • Ability to push and pull equipment weighing more than 40 lbs using proper techniques and assistance when necessary.
  • Manual dexterity, coordination and skillful use of hands when working with patients and equipment.
  • Visual acuity with the ability to distinguish colour.
  • Ability to perform documentation electronically and in writing.

Pay range: $29.39 - $48.20 per hour.
