Accurately assesses residents' health status as well as their educational and environmental needs on admission and on an ongoing basis. Communicates any significant change in condition to the appropriate interdisciplinary team (IDT) member. Accurately documents all assessments. Develops dynamic care plans based upon identified needs of residents through an interdisciplinary approach that includes physicians, residents, families, and IDT members. Reviews and revises the care plan as needs, problems, and goals change. Communicates the plan to IDT members along with residents and families. Documents the care plan on admission, cognition change, or regulatory requirements for care planning.
Identifies necessary nursing interventions which include nursing orders, physician orders, and residents/families educational and environmental needs. Interventions such as medication administration, treatments, and IV fluid administrations are performed safely and timely. Documents all interventions accurately and timely. Uses critical thinking when addressing changes in condition. Notifies the physician in a timely manner using the SBARR communication tool. Maintains composure during crises and professional interactions. Monitors care delivery to ensure the plan is followed. Uses the QAPI process to address issues for immediate problem resolution. Remains knowledgeable about resident status to identify behavioral changes or other signs. Assists in answering call lights and providing resident care such as ADLs. Completes ongoing nursing offerings to maintain professional development. Upholds commitment to current best nursing practices through knowledge of Bethesda policies and self-study. May be assigned other duties as appropriate.
Qualifications
- Education from an accredited school of nursing resulting in eligibility to become a registered nurse
- Current RN license in the state of practice
- CPR/AED certification required, or willingness to obtain within six months of employment
- Long-term care experience preferred
- Critical thinking skills
- Excellent communication skills (verbal) with patients, visitors, and staff
- Excellent time management and organizational skills
- Ability to stand and walk for long periods
- Ability to lift heavy objects with proper lifting techniques and assistance as necessary
- Ability to transfer and move patients weighing 200+ lbs with appropriate assistance
- Ability to push/pull equipment weighing over 40 lbs with proper techniques and assistance as needed
- Manual dexterity and coordination for working with patients and equipment
- Visual acuity with the ability to distinguish colour
- Ability to document electronically and in writing
Pay range: $29.39 - $48.20 per hour
