- Summary
Provides patient care support including hygiene, nutrition, ambulation, elimination, safety, and related activities as directed by the Charge Nurse.
Documents in the patient record per guidelines.
Performs resident orientation, responds to call lights, completes rounds, and assists with positioning, vital signs, intake/output, and equipment as ordered.
Assists with procedures and treatments as instructed.
Maintains supplies and equipment, and communicates with Charge Nurse regarding needs.
Participates in continuing education and accepts reassignment as needed.
Maintains confidentiality and professional relationships.
Attends end-of-shift reports.
Responsibilities
- Take and record vital signs and weights; report to Charge Nurse promptly.
- Assist with hygiene, nutrition, mobility, elimination, and safety per guidelines.
- Ensure proper patient identification, armbands, admission kits where applicable.
- Apply physician or therapy orders devices (slings, splints, braces, etc.).
- Accurately document in patient records; manage I&O sheets.
- Assist with procedures and treatments per instruction (e. g., Sitz baths, moist packs, isolation techniques).
- Maintain unit supplies and equipment; coordinate with purchasing when needed.
- Provide hand-off reports to oncoming staff; participate in rounds.
- Communicate effectively with team members and management.
Qualifications
- High school diploma preferred
- Active Certified Nursing Assistant certification in the state of employment
- CPR certification or willingness to obtain within six months
- Long-term care experience preferred
- Excellent communication skills; able to interact with patients, visitors, and staff
- Ability to manage time and meet productivity standards
- Ability to stand and walk for long periods
- Ability to lift and move patients with proper technique; weight considerations as needed
- Ability to push/pull equipment over 40 lbs with assistance
- Manual dexterity and coordination for patient care and equipment
- Ability to complete documentation accurately
- Visual acuity; color distinction
