Job Summary

As a Practice Nurse, you will play a vital role in delivering high-quality, patient‑centred care within a primary care setting. Key Responsibilities

Clinical care.

  • Conduct comprehensive health assessments, including taking medical histories, performing physical examinations, and ordering diagnostic tests.
  • Develop and implement individualised care plans based on patient needs and clinical guidelines.
  • Provide wound care, administer medications and vaccinations, and assist with minor surgical procedures.

Chronic disease management.

  • Manage care for patients with chronic conditions such as diabetes, asthma, COPD, and hypertension, ensuring regular monitoring and treatment adjustments.
  • Educate patients on self‑management techniques and lifestyle changes to improve their health outcomes.
  • Lead preventative screening programmes, such as cervical cytology, NHS Health Checks, and immunisation clinics.
  • Promote health education and awareness, providing advice on diet, exercise, smoking cessation, and alcohol reduction.

Patient education and support.

  • Empower patients with the knowledge and skills to take control of their health and well‑being through one‑on‑one education sessions and group workshops.
  • Provide compassionate and empathetic support to patients and their families, addressing concerns and answering questions.

Team collaboration.

  • Work collaboratively with GPs, healthcare assistants, and other healthcare professionals to deliver coordinated and integrated care.
  • Participate in team meetings, case conferences, and quality improvement initiatives to enhance service delivery.

Documentation and compliance.

  • Maintain accurate and up‑to‑date patient records using electronic health record systems (e.g., EMIS or SystmOne).
  • Ensure all clinical activities comply with NHS policies, Care Quality Commission (CQC) standards, and general data protection regulation (GDPR) requirements.

Person Specification

Qualifications & Experience

  • Registered nurse (RN) qualification with active NMC registration.
  • Additional qualifications in areas such as immunisation, cervical cytology, or chronic disease management are desirable.
  • Minimum of two years of post‑registration experience, ideally in a primary care or general practice setting.
  • Experience in chronic disease management and delivering preventative healthcare services.

Skills and Competencies

  • Strong clinical assessment, diagnostic, and treatment skills.
  • Excellent communication and interpersonal skills, with the ability to build rapport with patients and work effectively within a team.
