Patient Pathway Coordinator in Preston

The patient pathway coordinator (PPC) is an integral part of the patient pathway for NHS referrals (referral to discharge) ensuring that the process is both efficient and effective.

This is achieved by accurately processing and actively monitoring the flow of information throughout the NHS patient pathway.

Main duties of the job

• Work with clinical colleagues to ensure that NHS activity (clinics, procedures and diagnostics) is arranged safely, effectively and in a timely manner.
• For new referrals, review referral information provided by the GP / referring NHS Trust, pick out investigations or tests that have already been completed and ensure results are available and reviewed in a timely manner.
• Ensure that any required diagnostic tests are arranged in a timely manner, obtain and share results with relevant parties as soon as they are available, escalating any abnormal results immediately.
• Ensure results are added to patient record and take necessary follow-up actions, including removing patients from waitlists, booking follow-up appointments where required, and ensuring that all relevant parties (e. g.

NHS Trusts) are informed of outcomes in a timely manner.

• For follow-up appointments, ensure that they are arranged in a timely manner, ensuring that all relevant diagnostic results are available for review.
• Maintain accurate and timely records within the relevant electronic patient record.
• Track NHS patient pathways from referral to discharge, highlighting and resolving queries as they are encountered, ensuring that patients, NHS referrers (including NHS Trusts) and consultants are kept fully updated to optimise patient, NHS Organisation and clinician experience.
• Maintain excellent working relationships with internal and external / NHS colleagues, consultants and other providers.
• Chase reports, results and other documentation working proactively and independently to achieve required outcomes.
• Work with the relevant clinical teams and the Bespoke Healthcare radiology manager to manage imaging requests as required.
• Manage multiple tasks, prioritising own workload and working with the team to ensure work is completed within target timeframes.
• Work with GLH data officer and actively contribute to information required for local and national NHS data submissions including referral to treatment (RTT) waiting time data and diagnostic waiting time targets (DM01).
• Undertake any other reasonable duties commensurate with the nature of this role and any mandatory training as appropriate.

Experience

• Good organisational skills and a systematic approach to work with care taken over detail and due regard to confidentiality.
• Able to work independently and act on own initiative, seeking support and advice as required.
• Experience of working within a patient / customer focused environment.
• Self-motivated and resourceful, with the proven ability to multi-task and operate successfully under tight deadlines and time pressures.
• Enjoys a fast pace, dynamic, challenging and team-orientated work environment.
• Computer literate in MS Office e. g. Word, Excel and other window-based programmes.
• Strong writing, listening and speaking communication skills.
• Excellent interpersonal and teamwork skills.
• Experience of NHS booking systems and processes.
• Good understanding of National NHS waiting times (RTT and DM01).

Qualifications

• GCSE Grade A to C in English and Maths.
• Disclosure and Barring Service Check
