Community Nursing Team Lead in Bracknell

We are seeking an experienced and motivated Community Nursing Team Lead to play a key role in the success of our Community Nursing Teams. This leadership post is for an NMC-registered nurse with a Specialist Practitioner Qualification (SPQ) who demonstrates the Trust's values and behaviours.

You will lead, develop and operationally manage a designated Community Nursing Team, ensuring the delivery of high-quality, evidence-based, person-centred care across a seven-day service. This includes managing capacity, prioritising visits and responding effectively to service pressures.

The role involves supervising, supporting and developing staff, promoting interdisciplinary working, professional development and lifelong learning. Working closely with the Clinical Lead and stakeholders, you will support service integration, maintain CQC standards, manage delegated budgets, oversee recruitment and retention, and ensure compliance with policies, performance targets and contractual requirements.

You will demonstrate strong communication skills, sound clinical judgement and the ability to manage risk, investigate incidents and complaints, and embed learning across the service. You will maintain your own clinical competencies, NMC registration and professional portfolio while providing visible clinical leadership to teams managing complex caseloads. • Provide clinical and operational leadership for a Community Nursing Team, ensuring high standards of safe, person-centred care.
• Manage day-to-day service delivery, including capacity planning, prioritisation of visits, seven-day service cover, and escalation of risks or pressures.
• Lead, supervise and develop staff through appraisal, performance management, clinical supervision, education and training.
• Recruit, retain and support staff wellbeing, attendance, and professional development.
• Ensure compliance with CQC, organisational policies, clinical governance, safeguarding, health and safety, and risk management requirements.
• Manage delegated budgets, resources and staffing efficiently.
• Maintain effective communication within the team and with internal and external stakeholders, positively representing the service.
• Investigate complaints, incidents and serious incidents, identifying root causes, implementing improvements and sharing learning.
• Contribute to service improvement, quality assurance, audits, data reporting and strategic development of community nursing services.
• Maintain professional registration, clinical credibility and ongoing personal and professional development in line with NMC standards.
• Provide clinical leadership, support and guidance for complex caseloads, triage and evidence-based community nursing practice.

Education/Qualifications/Training

• NMC Registered Nurse

• Advanced History Taking and Community Nursing Prescriber (V100/150/300)
• Experience of coaching.
