To enhance the holistic wellbeing and quality of life of care home residents. Deliver meaningful, person-centred activities for individuals aged 65+, including those living with dementia and a range of physical abilities. Provide compassionate support, enabling residents to maintain their interests, social connections, and sense of identity and purpose. Deliver a varied programme of 1-to-1 and group activities tailored to each resident. Work closely with the wider care team to support residents’ emotional, social, and physical wellbeing, ensuring activities are inclusive, stimulating, and aligned with best practice, agreed standards, and available resources.

Main duties and responsibilities

  • Ensure residents are at the heart of all activities, promoting choice and socialisation.
  • Deliver person-centred activities that reflect residents’ life histories, preferences, routines, and abilities.
  • Provide support on a 1-to-1 basis and in small groups, particularly for those requiring additional support or living with dementia.
  • Demonstrate patience, compassion, and empathy in all interactions, building trusting and respectful relationships.
  • Plan, organise, and deliver a varied and engaging programme of activities, including local outings, community engagement, board games, reminiscence, reading aloud, chair-based exercises, arts and crafts, and seasonal celebrations.
  • Adapt activities to suit residents with different physical abilities and cognitive needs.
  • Support residents to express preferences, maintain routines, and continue hobbies and interests.
  • Provide companionship and emotional support, helping to reduce loneliness and social isolation.
  • Escort residents to medical appointments, supporting with manual handling and communication needs.
  • Support residents with nutritional intake, including assistance with food and drink.
  • Support residents to access communication methods such as phone calls, video calls, and letter writing.
  • Assist with some aspects of daily living where appropriate, in line with care plans.
  • Encourage participation in gentle physical activity to support health and mobility.
  • Work collaboratively with the multidisciplinary team to maximise opportunities for residents and ensure activities align with individual care plans.
  • Communicate effectively with colleagues, residents, families, and external partners, developing and maintaining positive working relationships.
  • Plan, deliver, and evaluate activities based on resident feedback and budget.
  • Research and introduce new ideas to enhance the activity programme.
  • Maintain accurate records of activities, participation, and outcomes where required.
  • Maintain skills at a current level and undertake training and development as required, including dementia awareness and best practice.
  • Attend all mandatory and role‑specific training as directed by management.
  • Promote and safeguard the welfare and wellbeing of all residents at all times.
  • Work in accordance with policies, procedures, legislation, and best practice.

Person requirements

  • A caring, kind, patient, and empathetic approach.
  • Understanding of person-centred care and holistic wellbeing.
  • Awareness of the needs of older people, including those living with dementia.
  • Ability to engage and support individuals 1-to-1 and in small groups.
  • Good communication skills, with the ability to adapt communication styles.
  • Ability to build positive relationships with residents, families, and colleagues.
  • Creative and flexible approach to planning and delivering activities.
  • Physically able to support residents, including pushing a wheelchair safely.
  • Basic IT and organisational skills.
  • Ability to work independently and as part of a team.
  • Willingness to learn and undertake training.
  • Experience working in a residential care home or similar setting (desirable).
  • Experience supporting individuals with dementia or additional needs (desirable).
  • Understanding of activity-based care and engagement strategies (desirable).
