Job Summary
We are seeking a highly motivated and organized Care Manager to join our team. In this role, you will coordinate patient care, collaborate with healthcare professionals, and ensure optimal patient outcomes. This position requires strong communication, organizational, and problem-solving skills, as well as a passion for patient-centered care.
Responsibilities
- Patient Care Coordination:
- Collaborate with the client, their family, agency team, and other healthcare professionals to develop and implement comprehensive patient care plans.
- Monitor client progress, identify potential problems, and adjust care plans as needed.
- Provide education and support to clients and their families regarding their health conditions and care plans.
- Communication and Collaboration:
- Maintain clear and effective communication with clients, families, and agency staff.
- Facilitate communication and collaboration among different departments and disciplines.
- Participate in team meetings and rounds to ensure coordinated care
- Documentation and Record Keeping:
- Maintain accurate and up-to-date client records and documentation.
- Ensure compliance with relevant regulations and standards.
- Resource Management:
- Identify and access appropriate resources for clients and their families.
- Advocate for client needs and ensure access to necessary services.
- Other Duties as Assigned:
- Perform other duties as assigned by the Agency Administrator and Agency Director.
Qualifications
At least two years of experience in care management, healthcare, or home care settings is preferred. Licensed Vocational Nurse Social Worker.
Skills
- Strong communication, interpersonal, and organizational skills.
- Ability to work independently and as part of a team.
- Proficiency in electronic health record (EHR) systems.
- Knowledge of relevant healthcare regulations and standards.
- Empathy, compassion, and a patient-centered approach.
Compensation: $45,000.00 - $55,000.00 per year

We offer medical insurance, life insurance, and Aflac supplemental coverage. We also have something most agencies don’t: a full-time social worker on staff, available to you at no cost for resources, referrals, or just support when you need it. She leads monthly in-service sessions focused specifically on caregiver wellbeing, because we know this work can be demanding and we want to make sure you’re taken care of too.
Compensation
Your starting pay is based on your experience and skill set, with CNAs receiving an additional bump. From there, your pay is not fixed: as you complete training levels and grow your skills, your hourly rate grows with you. You also earn PTO through completing training, and after one year you’re eligible for our 401k with a matching program. If you refer another caregiver who joins the team and works 80 hours, you earn a $250 referral bonus.
